Send your appointment emails to... parksquarevision@gmail.com Include your name, the date, preferred time you would like to come in and the appointment type: contacts, glasses or a pre/post lasik surgery appointment.

Also include the insurance type and/or separate vision insurance you will be using or if you will be private pay.

If your vision insurance is VSP or Eyemed PLEASE include date of birth and the last four numbers of the member's social security so that we can have all your information ready when you come in.

We will be happy to email you a confirmation of your appointment, or if the time is unavailable work with you to find a time that will work.
